Student Complaints
Students are encouraged to provide feedback — including complaints — that helps to make ULSD a better environment for individuals and the entire school community. While providing feedback directly to a faculty member, academic area or administrative unit is welcome, students may also use the links on this page to submit an online complaint directly to the School of Dentistry Dean's Office and/or University of Louisville Dean of Students Office.
If you need to report an urgent matter related to health or safety, please do not use these forms. Call 9-1-1, contact University of Louisville Police or go to the Dean's Office (School of Dentistry Suite 240) right away.
Ways to submit a student complaint online
UofL School of Dentistry Online Student Complaint Form
Use this form to submit a complaint directly to the ULSD Dean's Office. This form automatically fills in your contact information, so you can submit it quickly and easily.
UofL School of Dentistry Anonymous Submission Form
This form also goes directly to the ULSD Dean's Office, but it does not automatically fill in your contact information (unless you add an attachment).
University of Louisville Online Student Complaint Form
If you believe your rights have been violated, you can use this form to contact the University of Louisville Dean of Students Office directly.
